Each family will receive an envelope containing essential medical information that is required to be in compliance with the Houston Health Department. This information has to be updated and brought back to the Front Office by the Early Bird deadline date of February 29th.
1. Open the envelope and please proceed to update ALL Medical Requirements per student:
Medical Authorization Forms: This form must be completed and signed by a physician.
Immunization Record Document: Ensure your child's vaccinations are up to date. Please schedule a visit to your pediatrician promptly to avoid a lapse of the Early Bird Special.
2. Update TeacherEase Information:
Every field for every piece of information must be filled in including:
Emergency contacts addresses, new mailing addresses, phone numbers or emails.
Confirm consents
